APPLICATION FOR ADMISSION: JUNE 2009 NLE

The Professional Regulation Commission announces that application for admission to the June 2009 Nurse Licensure Examination is now being accepted until April 17, 2009 only. Applications may be filed at the Central Office at P. Paredes Street near Morayta (also at its satellite accepting center at the 2nd Floor of Ever Gotesco Mall – Recto Branch) in Manila or at any regional offices located in the cities of Baguio, Cagayan de Oro, Cebu, Davao, Iloilo, Legazpi, Lucena, Tacloban, Tuguegarao, and Zamboanga.

Applicants are required to submit the following:

1. Duly accomplished Nurses’ Application Form (NAF)
2. NSO issued Birth Certificate
3. NSO issued Marriage Certificate (if applicable)
4. Transcript of Records with scanned picture and reflecting the remarks: “FOR BOARD EXAMINATION PURPOSES”
5. Duly notarized Record of OR-DR Cases (for first-time applicants only)
6. Duly notarized Summary of Related Learning Experience (for first-time applicants only)
7. Four (4) passport size (1½ x 1 ½ ) colored picture with complete name and white background
8. Recent Community Tax Certificate (CTC)
9. P900.00 examination fee
10. One (1) window mailing envelope

Applicants are encouraged to file their applications early and not to wait for the deadline to avoid inconveniences brought about by long queues and overcrowding.
